Two sites reporting zero incidents may still have very different exposure and prevention controls. This project discussion scope uses ISO 45004 to review indicator definitions, sources, and interpretation so reporting can support control and resource decisions.
When to consider this work
Organizations with incident, inspection, or training data that does not help managers set priorities can start with existing reports. Review also helps when consolidating multiple sites, revising objectives, or introducing controls.
Common measurement challenges
Ranking sites only by incident counts may ignore differences in hours and risk. Treating more reports as worse performance may discourage reporting. Accurate calculation matters, as does the behavior an indicator encourages.
Guidance and applicable requirements
ISO 45004:2024 provides guidance on monitoring, measurement, analysis, and evaluation of OH&S performance, including indicator development. It can be used independently or within ISO 45001; an increased score does not establish certification. See the ISO catalogue.
Suggested work stages
-
Ask managers and OH&S staff what decisions lack information, then review whether current measures answer those questions.
-
Trace selected figures to source records and document formulas, duplicate entries, and site differences.
-
Pilot indicators for a defined period; note conclusions supported by evidence and limits caused by sample size or data quality.
-
Set regular data submission, review, correction, and management discussion so findings lead to actions.
Information to prepare
OH&S objectives and decisions managers need to make; indicator formulas, sources, denominators, frequency, and site scope; de-identified hours, incidents, inspections, and control records; previous corrections, meeting decisions, and follow-up actions.
Possible outputs
Possible outputs include an indicator dictionary, data-gap list, revised reporting method, and action follow-up schedule. Confirm the scope and deliverables before work begins.
